Applying for Benefits
You can request an application for benefits online through your account at
myplan.johnhancock.com
or by calling John Hancock at 1-855-312-CRCC (Para Español: 1-888-440-0022), representatives are available Monday-Friday 7 a.m. to 9 p.m.
Your completed application for benefits should be returned as directed on the forms package. If you are missing information on the form or if the required supporting documentation, if applicable, was omitted, it may be denied and then you will need to apply again. In some cases, the Fund Office will review your request and must approve the application before a distribution occurs. Once your completed application is received, please allow a minimum of 10 business days for your application to be processed.
