If you are enrolled in Comprehensive Medical Benefits or the Medicare Advantage Prescription Drug (MAPD) Plan and you die before age 65, your beneficiary is eligible for a life insurance benefit. Life insurance coverage for the Retiree Plan is underwritten by ULLICO, certain policy restrictions may apply. ULLICO is solely responsible for approval of claims.

The benefit is $25,000 which is fully insured and will be payable in a lump sum to your designated beneficiary. You may designate one or more beneficiaries by completing a Beneficiary Designation Form and returning it to the Fund Office. If there is no designated life insurance beneficiary on file, then the benefit will be paid to:

  • Your surviving spouse; or if none, then
  • Your surviving children, including adopted children, in equal shares; if none, then
  • Your surviving parents (in equal shares); if none, then
  • Your surviving biological brothers and sisters, in equal shares; if none, then
  • Your estate.
