INITIAL ELIGIBILITY APPRENTICE
You are eligible for benefits on the first day of the coverage quarter following a period (up to 6 months) when you worked at least 400 hours in Covered Employment and your employer submitted contributions on your behalf to the Fund Office. You qualify for (full) Active Plan benefits if you meet the Active Plan Requirements.

Once you are eligible, you will receive a Summary Plan Description booklet describing your benefits in more detail and certain forms you must complete will be mailed to your home. Send these completed forms to the Health Benefits Department:
CONTINUING APPRENTICE ELIGIBILITY
To continue your eligibility for benefits, you must work at least 200 hours in covered employment in a calendar quarter to remain eligible for the next coverage quarter.

Or if you have credit of at least 960 hours in the current and three immediately preceding calendar quarters, you will remain eligible for benefits for the next coverage quarter. If you lose your eligibility for benefits, you have options to continue your coverage.
If you are dropped from enrollment in the Apprentice Program, you are no longer eligible for coverage and may only maintain your current level of Plan coverage through continuation coverage under COBRA.
